Where the thumb is flicked from the top teeth.. unless I've got it wrong...

As for the two finger gesture, I always thought that it went back to the middle ages.
Whenever an archer was captured, his first two fingers would be chopped off rendering him an archer no more.
The two fingers was a gesture to the opposition to say 'ha,you haven't got me,I've still got my fingers'
