Kidney stones: Big ones. They actually hurt more (and for a longer time) than severing my finger tip.
An absessed tooth would be a distant (but still god awful) third.

A doctor should always eliminate the simple stuff first, which he could have done by taking urine, blood and prostatic fluid samples. The latter can be examined immediately for indications of infection. Acquiring the fluid means he has to stick a gloved hand up your ass in order to massage the prostate, but that's a hell of a lot less painful and traumatic than the procedure you're describing.

To make a long story short, luckily for me it was a prostate infection, whew! But for almost a week after the visit taking a piss was hurt like hell.
