There are some phrases that need to be included in this list:

-a whole 'nother
-all of a/the sudden
-I/we/she/he went and got

Also, in the grocery store "express" lanes, I commonly see the term: (x) items or less, which drives me nuts. It should be (x) items or fewer.
More of a grammar thing, but it should still be banned.

People who say "I'm good," instead of "I'm well."
Again, a grammar issue as opposed to a "word" issue. And although saying "I'm good" isn't technically incorrect, it still sounds awful.
